Job Opportunity: Support Worker in Intermediate Care Team
An exciting opportunity has arisen for an experienced Support Worker to join our Intermediate Care Team as part of the Integrated Community & Support Services (ICASS) in the St Andrews Locality.
The Intermediate Care Service provides rehabilitation to the adult population within the locality, delivered to service users as in-patients, out-patients, or within their own homes.
The primary role involves working with Occupational Therapy, Physiotherapy, and District Nursing Services within Intermediate Care to deliver therapeutic treatment programs. This includes working in ward settings or patients' homes to prevent hospital admissions and support early discharge. The specialist assessments are provided by Occupational Therapists, Physiotherapists, and District Nurses, who will allocate work accordingly. This role mainly focuses on daily community care, operating 365 days a year from 0800 to 2200.
Applicants should have an SVQ Level III in Health and Social Care or equivalent. Experience as a support worker is desirable, especially with older people, who are the primary service users. You will work as part of a multidisciplinary team, carrying out ongoing treatment programs for a mixed caseload.
Key skills include good organisational abilities, excellent communication, and the capacity to work independently and within a team. Flexibility to work across different community hospital sites is required. The role involves 15 hours of flexible work over 7 days.
We offer regular supervision, Personal Development Planning (PDP), and training opportunities. The ability to travel throughout the area is essential, as this is a community-based position.
