Clinical Pharmacist – Job Description
Role Summary:
We are seeking a proactive and experienced Clinical Pharmacist to join our multidisciplinary team. The successful candidate will play a key role in medicines optimisation, patient safety, and continuity of care, particularly in the context of long-term condition (LTC) management and polypharmacy.
Key Responsibilities:
· Conduct comprehensive polypharmacy reviews to ensure safe, effective, and evidence-based prescribing.
· Identify and implement alternative medications when standard options are unavailable, including liaison with specialists as needed.
· Adjust prescriptions to alternative formulations (e.g., liquid forms) to meet patient-specific needs.
· Provide cover for LTC pharmacist duties during periods of leave, including:
· - Reviewing incoming blood test results related to LTC reviews.
· - Actioning results in line with practice protocols and clinical guidelines.
Essential Skills & Attributes:
· Strong clinical knowledge and experience in primary care pharmacy.
· Excellent communication and collaboration skills.
· Ability to work independently and manage clinical priorities.
· Familiarity with EMIS or similar clinical systems is desirable.
