About the Role Join our Social Services Equipment and Adaptations Team as an experienced Occupational Therapist and embark on an engaging journey to enrich the lives of individuals across Stockport. In this role, you will be responsible for conducting comprehensive assessments and reviews, ensuring adults in our care receive the necessary equipment and adaptations to enhance their daily living and care needs. What you will do: - Undertake full Occupational Therapy assessments and reviews for each case referred to the Equipment and Adaptations Team. - Review existing packages of care for clients and liaise with neighbourhood teams to ensure care plans remain appropriate and up to date. - Complete full assessments for clients on the current EAAT (Equipment and Adaptations Assessment Team) waiting list and implement any necessary recommendations. - Conduct moving and handling assessments where required, provide suitable recommendations, and develop personalised moving and handling plans. - Review any adaptations previously completed following earlier OT involvement to ensure ongoing suitability and effectiveness. - Work collaboratively with multi-disciplinary teams to support client needs and promote independence. - Maintain accurate and timely documentation and records in line with professional standards and organisational policies. Person Specification: - Registered with the Health and Care Professions Council (HCPC). - Minimum of 2 years' experience as an Occupational Therapist, preferably with experience in equipment and adaptations or adult social care. - Ability and willingness to travel between multiple sites; driving licence and access to a vehicle preferred. - Excellent communication and interpersonal skills to liaise effectively with clients, families, and multi-disciplinary teams. - Commitment to delivering high-quality, person-centred care. - Ability to work independently and as part of a team.
