Are you passionate about helping people improve their wellbeing and create positive change in their lives? We're looking for a motivated and compassionate Social Prescribing Link Worker to join our Greater Middlesbrough Social Prescribing Team, with a dedicated focus on supporting people experiencing complex mental health needs.
Working across GP practices within the Greater Middlesbrough Primary Care Network, you'll provide personalised, holistic, and trauma-informed support to individuals who may face barriers to accessing services or experience health inequalities.
Main duties of the job
* Work one-to-one with people to explore their strengths, goals and the challenges they face.
* Connect individuals with community groups, activities, and services that enhance their wellbeing, confidence and social connections.
* Support people to develop coping strategies, improve emotional resilience and navigate difficult circumstances.
* Work in partnership with primary care and community organisations to ensure the best outcomes for those you support.
* Help identify gaps in local provision and contribute to the shaping of future services.
You'll need to be caring, enthusiastic and highly organised, with experience of working in trauma-informed ways and supporting people with complex or multiple needs. Strong communication skills, the ability to work independently, and a commitment to empowering individuals are essential.
