Overview
The post holder will work within the Community Services Division using advanced skills to identify, assess, diagnose, treat and manage patients with palliative care needs. You will work in patients\' own homes, care homes and hospital settings, seven days a week, as part of a multidisciplinary team with high accessibility within the division. The post holder will rapidly assess patients known to the community specialist palliative care team who deteriorate unexpectedly at home or in care homes, to help make decisions in line with the patient\'s wishes and negotiate a management plan based on appropriate ceilings of care. This may lead to hospital or hospice admission, enhanced interventions at home under a virtual ward, or admission under hospital or community-based support with assistance from advanced clinical practitioners.
In hospital, the post holder will screen patients presenting to admitting areas for unmet palliative care needs, which may lead to ongoing hospital admission with support from the hospital specialist palliative care team and potential rapid discharge home, hospice admission, or admission home with enhanced interventions in a virtual ward environment.
Responsibilities
* Deliver high-quality patient care, monitor practice, challenge care where appropriate, and take proactive steps to identify patients who are seriously ill and to improve their experience by aligning care with their wishes.
* Lead and participate in formal and informal teaching across the community, neighbourhoods and hospital settings to improve palliative care knowledge and support for generalist staff.
* Provide and support advance care planning and education.
* Collaborate with the Matron for Specialist Palliative Care, advanced clinical practitioners in hospital and community, Consultants in palliative medicine, nursing staff, Allied Healthcare Professionals and support staff to enhance care at home and reduce unnecessary hospital admissions for patients in the last 90 days of life.
* Aim to prevent unnecessary hospital admissions, meet patient wishes and reduce length of stay where hospital admission is needed.
