About the Role
Position: Social Worker (Level 2)
Service: Adult Social Care continuing health care (CHC) at Bath and North East Somerset Council
The role supports the CHC Team Manager in carrying out CHC assessments and making joint recommendations for NHS funding. The successful candidate will identify adult care service users whose health care needs exceed those lawfully met through the Care Act 2014 and determine eligibility for full NHS funding or joint health and social care funding.
Key Responsibilities
* Identify adult care service user groups requiring health care needs above those lawfully met by the Care Act 2014.
* Carry out CHC assessments alongside a CHC Nurse Assessor.
* Make joint recommendations regarding each person’s eligibility for full NHS funding.
* Identify people whose needs could be met through joint health and social care funding.
* Participate in a full induction programme and ongoing professional development.

Additional Requirements
* Completed ASYE is preferred.
* Knowledge and experience of continuing health care.
* Enhanced/Basic DBS Check. The post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975; a submission for Disclosure will be made to DBSC.
* Prior consideration for roles for any individual on the Council’s at‑risk register.
