We are excited to offer a permanent Practice Consultant role within the West Place Physical Disabilities/Older People Team, covering Fens & Brecks area. This role will be based at Priory House in Kings Lynn. This is a unique opportunity to join us at a key moment of transformation as we embed a Community Led Support (CLS) approach across West Place. You will work closely with the Team Manager to support day to day leadership, drive improvement work and champion strengths-based, community-focused practice. You will: • Support the ongoing development and embedding of Community Led Support • Help create a confident, learning focused environment across the service • Strengthen relationships with local partners, voluntary groups and communities • Contribute to shaping local practice and exploring innovative approaches to support adults • Hold a caseload of complex cases We’re looking for someone who is: • Energised by improvement and new ways of working • Skilled in people leadership, coaching and developing others • Experienced within adult social care, community-based support or integrated care • A strong communicator who can bring people together and drive positive change • Passionate about strengths based, community led practice Whether you have previously been in a similar role or you're ready to step up, this is a fantastic opportunity to grow and lead meaningful work.
