Overview
We are looking for an enthusiastic and committed staff member to join our existing community service. The aim of the service is to provide recovery-focused treatment and support via personalised care plans for individuals facing a range of mental health disorders.
Team
You will work with the Multi-Disciplinary Team that includes Nurses, Psychologists, Doctors, Social Workers, Occupational Therapists, and support staff.
Key Responsibilities
* Support the Associate Practitioner with physical health assessments, including phlebotomy and participation in clinics such as the Clozapine Clinic.
* Visit service users in their home environments; provide support to carers and liaise with system partners.
* Complete clinical documentation following each service user contact; ensure progress and concerns are shared with the Multi-Disciplinary Team.
* Attend the weekly Multi-Disciplinary Team meeting.
* Report and document incidents as required.
* Provide education and support to develop Service Users’ understanding of mental illness and prevent relapse.
* Support the implementation of care packages and review them as required.
Qualifications
* Experience in a mental health or community care setting.
* Strong communication and interpersonal skills.
* Ability to work collaboratively with a multi-disciplinary team.
* Willingness to undertake mandatory training, including a 5-day face-to-face induction and additional corporate training.

Professional Development
Successful candidates will attend a 5-day face‑to‑face induction programme and further mandatory training. Additional training will be delivered face‑to‑face or virtually.
