We are looking for a highly motivated, experienced Advanced Clinical Practitioner to join our well-established clinical team for up to hours per week.
You would be joining a friendly and dynamic practice team who serve a practice population of over 13,500 patients and aim to deliver modern primary care with the traditional feel of general practice.
You will be supported by an excellent clinical team, a well structured management team and an outstanding administrative team.
Main duties of the job
In this role you will work independently, making professional,autonomous decisions.
The role will involve initial history taking,clinical assessment, diagnosis, treatment plan and evaluation of patient care.Assessment may be face to face by appointment or by telephone as part of atriage/unscheduled care team and will include patients presenting with anundifferentiated diagnosis.
You will be responsible for making any necessaryreferrals, requests for investigations and do these in in an appropriate mannerand timescale.
The role will involve independent prescribingwithin the scope of professional practice.
The advanced clinical practitioner will support theGP team managing same day appointment requests from patients and home visits.
The clinical practitioner will need to prioritiseand triage the needs of patients accordingly, seeking GP input as appropriatewhere the problem lies outside their professional capability. They willdemonstrate safe clinical decision making and a high level of independent careincluding assessment, diagnostic and prescribing skills.
All appointments are 15 minutes.
About us
Whitby Group Practice is a personal Medical Services Practice,located in Spring Vale Medical Centre in Whitby, North Yorkshire. It has acurrent list size of 13,500 patients.
The Practice provides a service to residents of Whitby and thesurrounding villages. It has a branch surgery at Robin Hoods Bay,approximately 5 miles from Whitby along the coast. Both surgeries are purposebuilt, have a dispensary and ample parking. They are owned by the Partners.
The Practice is a large team which works together to deliverhigh quality, accessible care within a supportive, friendly learningenvironment. The GP Partners have personal patient lists, and most workpart-time work in a job-share team with other Partners.

Clinical practice
1. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
2. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
3. Carry out face to face, telephone, video appointments and triage patients appropriately
4. Carry out home visits
5. Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs
6. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
7. Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
8. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
9. Work with patients in order to support compliance with and adherence to prescribed treatments
10. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
11. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
12. Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
13. Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation
14. Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health
15. Promote and deliver evidence-based care for patients presenting with aural conditions

26. Use a structured framework (eg root-cause analysis) to manage, review and identify learning from patient complaints, clinical incidents and near-miss events

29. Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patients health procedures and local guidance
30. Working with practice management to ensure the whole team have skills and knowledge regarding domestic violence, vulnerable adults, substance abuse and addictive behaviour. Provide guidance and support to ensure appropriate referral if required
