Salary: £44,845 - £46,992 (inclusive of 12.5% market supplement)

We are seeking an experienced Social Worker ready to step into a role that combines leadership and skill development.
Responsibilities
* Collaborate with individuals, families and caregivers to support and intervene for people aged 18 and over with physical disabilities, chronic conditions, autism and sensory impairment.
* Conduct Care Act 2014 assessments, evaluate needs and manage both short‑term and long‑term cases.
* Advance safeguarding inquiries, handle Court of Protection matters and maintain close collaboration with healthcare and multidisciplinary professionals.
* Provide monthly supervision and support professional and career development of team members.
Qualifications
* Registered Social Worker with Social Work England.
* Solid understanding of the Care Act 2014.
* Experience in safeguarding and Court of Protection matters preferred.
* Positive, risk‑focused approach and a passion to make a difference.
