Role Overview
We are seeking a skilled occupational therapist to join the CLCH ICSS multidisciplinary team, providing a comprehensive service to residents across West Hertfordshire, both in their homes and in community settings. The ICSS includes the Early Supported Discharge Team (ESD), Neuro Enhanced Transition Team (NETT) and the Community Stroke Team.
Responsibilities
* Work autonomously within professional standards and clinical guidelines, promoting best practice while managing your own caseload.
* Assess, treat and document care for patients/clients in line with service protocols.
* Build and maintain strong professional relationships with referrers and partner services, including GPs, acute providers, Bedded Services, District Nurses, Wheelchair Services and other CLCH teams, to ensure timely and appropriate referrals.
* Supervise rehabilitation assistants and support occupational therapy students, contributing to their professional growth.
* Conduct multidisciplinary neurological assessments to identify clients’ needs and collaborate with the MDT on joint goals.
* Perform specialist occupational therapy assessments, including functional, cognitive, visual, vocational, seating and upper limb assessments, and identify priorities for patient-centred interventions.
* Work collaboratively with other healthcare professionals to ensure holistic care planning and delivery.
* Maintain accurate, timely and professional documentation for all assessments, interventions and communications.
* Actively participate in CPD, service audits and research projects to support continuous improvement and evidence-based practice.
