We are looking for an enthusiastic Pharmacist who will embrace working within the community setting within Swindon. We are looking for a Senior Pharmacist (band 8a) but would also consider a development position (band 7-8a) if this is something you are looking towards.
You will be a Senior Pharmacist supporting the exciting Urgent Response and NHS@Home service in Swindon which incorporates Virtual Wards. A virtual ward is a safe and efficient alternative to NHS bedded care providing treatment and care in a patient's own home. This supports early discharge out of hospital and prevents avoidable admissions. You will provide a highly specialist clinical pharmacy service to virtual ward patients, optimising medicines and ensuring agreed changes in medication are implemented and sustained. This could involve remote consultations, home visits and attending multidisciplinary team meetings. You will be involved in developing and delivering safe and effective pathways of care to patients for a range of conditions. You will also be a key part of a collaborative approach to virtual wards across the BaNES, Swindon and Wiltshire (BSW) Integrated Care Board (ICB).
As a Senior Pharmacist you may be required to support the wider pharmacy team within Great Western Hospitals as the needs require. This may include clinical pharmacy ward and dispensary support. Please see the job description for further details.
Main duties of the job
1. Carry out medicine reviews virtually and face to face in patient homes, as necessary
2. Work as an independent prescriber (when trained), optimising medicines and promoting appropriate evidence-based and cost-effective prescribing
3. Utilise clinical assessment skills, where competent and appropriate, to support clinical review of patients within the Virtual ward
4. Provide professional advice and support during consultant and multidisciplinary Virtual Ward rounds
5. Assist in the management of prescribing across the primary/secondary care interface to promote a unified and coordinated approach
6. Developing the pharmacy virtual ward service, in line with national standards
7. Support patients and their carers in respect of their medication, including education for patients/carers and medicines advice to health and social care providers
8. Support and develop the admission and discharge process to and from Urgent Response and NHS@Home services to ensure safe transfer of care in relation to medicines.
9. Co-ordinate the day-to-day activities of the pharmacy service, supervising and line managing pharmacy technicians and rotational pharmacists within the service.

11. Responsibility for leading pharmacy input into the Urgent Response and NHS@Home service at the Great Western Hospitals NHS Foundation Trust
12. Support and address a range of pharmaceutical care needs for patients, such as prescribing or deprescribing, medicines reconciliation, medication review, medicines adherence assessments, ensuring timely medicine supply, antimicrobial stewardship, and therapeutic drug monitoring for patients within these services
13. Support the delivery of other domiciliary medicines optimisation services as new care models emerge and participates in the delivery of other community clinical pharmacy services as required
14. To deputise and support the Lead Pharmacist for Integrated Community Services

15. Registration with General Pharmaceutical Council
16. Postgraduate qualification in clinical pharmacy or equivalent experiential practice
17. Independent Prescribing annotation or desire to complete qualification
18. Keeping professionally up to date including CPD

20. Relevant national and regional training courses
21. Member of the Royal Pharmaceutical Society of Great Britain
Experience

22. Practice as clinical pharmacist.
23. Experience of managing a team.

25. Proven track record of service development.
