Overview
We are looking for a talented and highly motivated individual who wants to make a real difference to our patients and staff, to join our team at Mersey and West Lancashire Teaching Hospitals NHS Trust as a Clinical Patient Safety Manager for the Division of Medicine and Urgent Care. We are seeking an inspirational, influential leader who is confident, dynamic, visible and has or would like to gain experience working at a senior level in healthcare, particularly in the management of Incident Investigations.

Responsibilities
* Incidents & Patient Safety: Provide patient safety and clinical risk advice to all members of the Trust; lead and support divisional investigations, ensuring learning, timely reporting, and alignment with Trust policies (including Datix).
* Governance & Investigations: Work with the Head of Risk and the Integrated Governance Management team to develop processes that demonstrate learning from complaints, clinical incidents and legal claims; ensure duty of candour where applicable; manage investigation timelines; line manage the Investigations Officer.
* Risk Assessment & Risk Register: Ensure risks within Integrated Governance are risk assessed and recorded on the Risk Register (Datix) and presented at appropriate forums.
* Safety Alerts & Policy: Disseminate patient safety alerts (e.g., MHRA) in conjunction with the Physical Risk Manager; contribute to the development of standard operating procedures and risk management policies; monitor compliance with actions and evidence.
* Communication: Present complex information to multi-disciplinary teams; liaise with patients and families when incidents involve them; support Harm Review Meetings and provide debriefing where appropriate; ensure timely and sensitive information handling.
* Organisational & Management Skills: Use computer systems (Microsoft Office); develop skills and support others; ensure smooth departmental operations in the Head of Risk's absence; line manage the Investigations Officer and oversee HR processes and appraisal.
* Training: Prepare training materials and participate in training delivery related to patient safety, risk assessment and investigation management; participate in performance appraisal and address barriers to completion with line management.
Risk Management & Incident Reporting
The post holder will:
* Produce quarterly reports for the Quality and Safety Committee reviewing incidents, trends, lessons learnt and changes to practice/service across the Trust and Business Units.
* Produce monthly lessons-learned bulletins with the claims manager and complaints manager from Serious Incidents and other sources.
* Produce Serious Incident review reports for internal and external stakeholders including patients and families; ensure action plans for StEIS and SUIs are implemented within deadlines.
* Develop incident reduction strategies in collaboration with Trust-wide groups and Business Units.
Safety & Compliance
* Ensure all risks are recorded and reviewed; manage risk in Datix; disseminate safety alerts; ensure actions are closed with supporting evidence.
Policy & Documentation
* Develop and maintain risk management policies; support Datix use; monitor quality of incidents.
Communication & Team Working
* Demonstrate high-level communication; liaise with multi-professional teams; support units in harmonising risk assessments and incident reviews; provide feedback and debrief to staff involved in incidents.
Organisational & Management Skills
* Proficient in MS Office; develop personal and team capabilities; prioritise investigations by risk and deadlines; ensure cover for annual leave; line manage the Investigations Officer.
Training
* Prepare and deliver training materials on patient safety and risk management; participate in performance appraisals and address barriers to completion in a timely manner.
Qualifications & Experience
Essential
* Clinical Professional Qualification (currently registered)
* Degree-level education with willingness to undertake further education as required
* Experience of managing a team; knowledge of clinical risk management; experience implementing risk management policies; experience in Root Cause Analysis/Investigations; experience with Datix or similar risk management systems
