At The Forward Trust, we are committed to supporting individuals in their recovery from substance misuse and mental health challenges. Were currently recruiting a Health and Wellbeing Practitioner to join our dedicated team at HMP Coldingley, a closed Category C male prison in Surrey.
As part of an integrated healthcare framework, youll work alongside a multi:disciplinary team to deliver a mix of clinical and psychosocial interventions to adult male prisoners. This includes individuals presenting with low:level mental health needs and/or substance misuse issues.
You will be responsible for:
* Managing a caseload and delivering tailored recovery:focused interventions
* Conducting assessments and co:producing SMART care plans with service users
* Facilitating structured 1:1 sessions and group programmes
* Providing harm reduction advice and practical recovery support
* Supporting release planning and ensuring through:care into community services
* Performing second signatory duties as needed
* Maintaining accurate, up:to:date case records in line with service KPIs
This is a 7:day:a:week service, so weekend working will be required on a rota basis.
