Job overview

We are looking for an enthusiastic and hard-working person to work across the Integrated Frailty Service in the Hospital at Home and Urgent Community Response within the Royal Surrey NHS Foundation Trust, based in Milford Hospital.

Main duties of the job

You will work as part of a multidisciplinary team alongside therapists, doctors, Advanced Care Practitioners, nurses, and pharmacists to provide integrated and personalised care to the patients.

We aim to provide hospital-based care in the home environment, avoiding unnecessary admissions to the acute hospital and supporting early discharges home.

We are a relatively new service with lots of opportunity for service and personal development.

You will receive an annual appraisal and personal development plan, enabling you to have a structure within your role and help you develop personally and professionally.

Working for our organisation

Royal Surrey is a compassionate and collaborative acute and community Trust. Recognising that our 5000 colleagues are our greatest strength, we offer a comprehensive health and wellbeing program along with a commitment to developing and advancing your career. Our diverse and welcoming Royal Surrey family will ensure you feel valued from your initial interview through your entire tenure.

We are clinically led and provide joined-up care by bridging the gap between hospital and community services alongside regional specialist cancer care. Our main acute hospital site is in Guildford with community hospital sites at Milford, Haslemere, and Cranleigh. We provide adult community health services in homes across Guildford and Waverley.

The Care Quality Commission (CQC) have given us an overall rating of Outstanding.

Royal Surrey has a strong reputation and history to build on. We are proud of our achievements and we are investing in our colleagues through our commitment to supporting professional development as well as investing more than £45 million in our physical environment and new equipment in the next few years. There has never been a better time to join us.

Although it isn't the Trust's normal practice, adverts may close early, so you are encouraged to submit an application as soon as possible.

Detailed job description and main responsibilities

The post holder will be a key member of the Integrated Frailty Multi-professional team including Consultant Geriatricians, Advanced Clinical practitioners (ACPs), Specialist registered Nurses, Paramedics, Senior Physiotherapists, Community Rehabilitation Practitioners, and a GP. The Team will provide hospital-based care at home and an urgent response to patients who are experiencing a health crisis with the aim to keep them in their own homes safely and avoid a hospital admission.

You will assess and treat referred patients into the Hospital at Home and Urgent Community Response team using evidence-based practice and clinical experience and specialist skills in this area. You will provide assessment and treatment to maintain patients in their own homes and stabilise those patients who have urgent care needs in the community and initiate services, which safely avoid unnecessary hospitalisation.
