Surrey Downs Health & Care Community Therapy Team
The Surrey Downs Health and Care Community Therapy Team works across organisational boundaries with GP Primary Care Networks, providing rapid access therapeutic/MDT intervention for those who would otherwise need emergency hospital admission. The team also delivers rehabilitation and falls prevention for patients in their own homes, working closely with rapid response services under the Urgent Community Response initiative to support people living independently and reduce admission to acute services.
Responsibilities
* Perform advanced physiotherapeutic assessment of patients with complex presentations, using advanced clinical reasoning skills to create targeted treatment plans with SMART goals specific to each condition.
* Formulate and deliver individual or group physiotherapy treatment programmes in line with best practice guidelines.
* Coordinate and ensure effective delivery of therapy services by liaising with colleagues across other PCNs and adult social care teams to provide equitable services.
* Evaluate patient progress, reassess and alter treatment programmes as required, advising and supporting junior colleagues and handing over treatment plans to rehabilitation assistants.
* Have awareness of local community services and refer to these appropriately and in a timely manner.
* Understand and manage clinical risk within own patient caseload and the service, advising junior staff and seeking guidance from Band 7s/Clinical Lead as required.
* Keep accurate, complete records of work undertaken in line with CSP and community therapy team standards, maintaining confidentiality and information governance.
* Teach, assess and contribute to performance assessment of Band 5s and rehabilitation assistants.
