Job Duties: Medication Reconciliation of clinical letters and Hospital Discharge Summaries Medicine Management including prescription screening and processing Supporting with the ‘Repeat Dispending’ process Responding and communicating to patient related medication queries Coordinating with local pharmacies to manage stock levels and shortages Medication synchronisation in line with regular repeat prescription To oversee medication recall processes, ensuring timely responses and compliance.
