Are you passionate about making a real impact in the lives of children and young adults with disabilities? We are looking for a committed and experienced Social Worker to join our dynamic All Age Disability Service, supporting children and young people aged 0-25 and their families.
The Role:
You'll work within a supportive multi-disciplinary team to carry out statutory assessments and deliver high-quality care and support planning under the Care Act 2014 and Children Act 1989. This includes working with children in need, child protection plans, and transitions into adulthood.
Key Responsibilities:
* Undertake complex casework and statutory assessments
* Develop personalised care and support plans
* Contribute to safeguarding investigations and attend child protection conferences
* Support young people in transition to adulthood, including preparing for independence
* Liaise with education, health, and voluntary sectors to ensure coordinated support
* Maintain accurate case records in line with professional and legal standards
About You:
* Qualified Social Worker with current Social Work England registration
* Minimum 2 years' post-qualifying experience (experience with 0-25 or disability services preferred)
* Strong working knowledge of Care Act 2014, Children Act 1989, and SEND code of practice
* Skilled in building relationships with families, young people, and partner agencies
* Resilient, reflective, and able to manage a diverse caseload
* Enhanced DBS required
