Job overview

An exciting opportunity has arisen for an experienced Clinical Nurse Specialist to join GSTT Hepatobiliary (HPB) CNS service, working within the GI HPB team working alongside the multi professional clinical and medical team. The successful candidate will work alongside a well-established team to deliver care for patients with liver, pancreatic, biliary and gallbladder disorders, both malignant and benign conditions.

The successful candidate will be expected to co-ordinate care for patients with suspected HPB cancers and be a point of contact and source of information. This will be through coordinatingsurveillance and care for patients on long term HPB related follow up

The role involves supporting Gastro and Upper GI surgical clinics, liaising with other Trusts within the sector regarding referrals and being expected to build good working relationships within the team and beyond.

Main duties of the job

The role includes preparing and manage the HPB MDMs (Benign and Malignant). This will include the documentation and patient follow up, booking of procedures and appointments etc.

The successful candidate will be expected to co-ordinating care for patient with suspected HPB cancers and providing a point of contact and source of information. This will be through co-ordinating surveillance and care for patients on long tern HPB related follow up (Eg. Pancreatic side branch IPMN, Gallbladder polyps)

The role involves supporting Gastro and Upper GI surgical clinics, liaising with King’s College Hospital regarding MDM referrals and building a firm relationship with this team.

The candidate will also be involved in advising, supporting and training staff across the trust regarding HPB conditions and management. They will be expected to be able to deliver and develop high quality educational sessions to meet the needs of the staff on the wards.

The successful candidate will need to promote the development of evidence-based practice, be involved in the development of national guidelines and protocols which are consistent, relevant and reliable. Be able to share information across the different regions and gain insight from others.

As part of the Gastrointestinal Medicine & Surgical directorate, the hepatopancreatobiliary (HPB) service delivers multidisciplinary care for patients with conditions affecting the liver, pancreas, gallbladder and biliary system. This includes both benign and malignant disease

Within this directorate, the role of the HPB Clinical Nurse Specialist (CNS) is central to the coordination and delivery of high-quality patient care. The CNS provides expert clinical knowledge, leadership and support across the patient pathway for patients and their families.

We believe that training, education and effective communication are key factors in lower limb care management.

Detailed job description and main responsibilities

·Develop and maintain effective communication with the wider MDT, patients and relatives/carers, in relation to delivering results and outcomes.

·The post holder would be expected to provide advanced specialist nursing care for patients and undertake comprehensive patient assessments including history taking, physical assessments and individualised care plans.

·The role would require the candidate to work independently as a specialist nurse including delivering nurse led clinics, drawing on their extensive experience to be able to use their professional judgment and autonomy in clinical decision making.

·The candidate will be responsible for responding timely through clinical triage of 2ww referrals, use of STT model where appropriate and assess patients via telephone or F2F clinics. Therefore, the applicant needs to have expert knowledge of upper GI anatomy, physiology and psychological functioning for this group of patients in order to produce an effective plan of care.

·The post holder will also need to have knowledge and experience of suspected upper GI cancer and benign endoscopy and diagnostic pathways, linking in with the MDM meetings and clinical teams. An understanding of the Faster Diagnostic Standard and Referral to Treatment specifications is also required. They will work very closely with the cancer pathway tracking teams and help maintain efficient timelines on the patient tracking lists (PTLs) for this group of patients. They should ideally be working at advanced practice level with advanced clinical assessment and non-medical prescribing qualification or willingness to do so.

·Excellent communication and documentation is essential as the role will include delivering sensitive diagnosis as well as seeing patients in a clinical setting and during procedures.

Patient advocacy and empowerment are essential for this post holder.

Strong clinical leadership is essential. The post holder will be expected to support the development and implementation of new clinical pathways, adherence to national policy and evidence-based practice. It will be expected for the candidate to lead service improvement initiatives, develop policies, audit and lead quality improvement projects.

1. Significant experience working with HPB/Upper GI/Oncology and Hepatology patients
2. Registered Nurse (RN)
3. Experience working autonomously
4. Experience of Clinical Leadership
5. Experience working as a Clinical Nurse Specialist
6. Working with the Multi Disciplinary Team
7. Experience with co ordinating complex patient pathways

Desirable criteria

8. Experience managing nurse led clinics
9. Experience in Service Improvement or Quality improvement incentives
10. Experience preparing for MDM and writing up outcomes

Education & Qualifications

Essential criteria

11. Degree and/or post graduate diploma/ or equivalent experience
12. NMC registered nurse
13. Post Graduate Education
14. Advanced Clinical Assessment qualification

Desirable criteria

15. Non medical prescribing qualification
16. Masters Degree in Nursing/Advanced Practice
17. Specialist training in Hepatology, Oncology, Palliative Care or Upper GI/HPB
18. Leadership qualifications/training
