Support carers through signposting, emotional support, advocacy, and by facilitating access to relevant services. Assist veterans and their families in navigating and accessing appropriate healthcare and community-based services. Promote self-management and independence by providing personalised support that enables individuals, families, and carers to take control of their health, make informed decisions, and improve health and wellbeing outcomes.
