Overview

The post holder will be welcomed as a member of the Community Learning Disability team in North Tyneside, with regular team meetings, robust support and supervision, and a commitment to continuing professional development. The post is based with the Community Learning Disability Team in North Tyneside and will involve working in a range of settings across North Tyneside.


Responsibilities

* Manage a personal caseload and work across primary and secondary healthcare services to ensure equity of access for adults with a learning disability.
* Assess, plan, implement and evaluate high quality care for adults with a learning disability.
* Support the STOMP (Stopping the overmedication of people with a learning disability) team in delivering assessment, Positive Behavioural Support (PBS) care planning and medication management, and provide STOMP training to parents, carers and wider stakeholders.
* Support the skills of the wider Community Learning Disability Team to ensure physical health and wellbeing is prioritised and embedded in all aspects of specialist work.
* Chair, attend and contribute to all relevant meetings; provide supervision and appraisal for junior staff; participate in training development and delivery; undertake group work; participate in audits and quality improvement projects.
* Provide specialised nursing care and Positive Behavioural Support plans for adults in the North Tyneside area and supervise junior staff within the Integrated Community Learning Disability Team to maintain high standards of care.
* Lead service development, audits and quality improvements as part of the role.


Qualifications and requirements

* Qualified nurse with current registration and relevant experience in learning disability services; supervisory experience is desirable.
* Be able to work independently and as part of a team; capable of chairing and contributing to meetings and training sessions.
* Full driving licence and access to a vehicle is essential.
* Applicants who are members of the Armed Forces, and those who have a disability requiring workplace support, will be interviewed under the Trust\'s interview guarantee scheme.
* Fit and Proper Person Requirements (FPPR) will require additional pre-employment checks in line with CQC and NHS England guidance.
* Willingness to read and follow applicant guidance notes before submitting an application.
