Key Responsibilities
* Perform high-quality, timely patient assessments and determine clinical judgment of the patient’s condition/needs.
* Develop, implement, and modify individualized treatment programs with a person-centered and goal-focused approach.
* Monitor and evaluate treatment plans to ensure maximum patient benefit and health outcomes.
* Ensure appropriate pathways of care and communication via liaison and referral to other agencies as needed.
* Work as an autonomous practitioner responsible for your own clinical caseload, including patients with complex needs.
* Access regular training, supervision, and support from senior staff.
* Rotate through various specialties, including Acute Stroke, Stroke Rehabilitation, Respiratory/Medicine, General Rehabilitation, Orthopedics, MSK, and Community.
