On site presence for part of the week is required. Open to applications from Children's services workers. Fast paced work The team supports a multi- disciplinary approach, with a focus on adults who have multi-faceted complex needs and cognitive decline. This service focuses on outcomes for individuals, maximising individual potential and minimising risk to independence, with an emphasis on choice and control.
