The Community Mental Health Nurse is a key clinical leader within the multidisciplinary care team, responsible for assessing, planning, coordinating, delivering, and evaluating care. The role involves communicating findings, influencing change, and promoting health, best practices, patient safety, and positive experiences among our caseload of older adults.
The post holder must hold a full, valid UK driving licence and have access to a car/vehicle for business purposes (unless you have a disability as defined by the Equality Act 2010).
1. Carry out and coordinate comprehensive, systematic nursing assessments considering mental, physical, social, cultural, psychological, spiritual, and environmental factors, in partnership with service users and others through interaction, observation, and measurement.
2. Work collaboratively with other disciplines, service users, and their carers to develop a holistic, person-centred care plan addressing identified needs.
3. Deliver and evaluate safe, person-centred care in partnership with service users and their carers to support recovery.
4. Act as a clinical leader supporting best practices in line with NICE guidance.
5. Facilitate and review approved Quality Improvement initiatives within the care setting.
