We are looking for an enthusiastic and motivated Band 6 Mental Health Practitioner to join our dynamic Intensive Support Function Team. In this role, you'll provide high quality, compassionate care to children and young people with a diagnosed Learning Disability and/or Autism Spectrum Condition who are at risk of admission to a mental health hospital and/pr placement break down. You'll work as part of a Multi Disciplinary team that construct holistic assessment and deliver specialist interventions to support Children, young people, their families and their wider support network.

The Intensive Support Function Team Cheshire is based within Crewe but supports Children and Young people across the entirety of Cheshire and utilises a hybrid working pattern to support with this.

This role affords practitioners to utilise an array of skills and encourages creativity and flexibility in clinical thinking.

Clinicians will work within a robust MDT supporting a shared caseload.

To work with clients experiencing mental health and or behavioural challenges, and their families, in the Intensive Support Function team. The post holder will hold a professional qualification and have relevant experience regarding children mental health care.

The Post Holder Will Be Responsible For

* Undertaking assessments and coordinating the care of service users.
* To work with family members closely to maintain high standards of collaborative care.
* To actively contribute to the day to day operational functions of the team such as completing new assessments, coordinating cases within the shared caseload and supporting junior team members.
* To develop and maintain positive working relationships with service users, their carers, education provision, third sector partners, other significant professionals, and members of the multi-disciplinary team.
* Ensuring that clear channels of communication are established and maintained with children and young people, families, carers, GPs, Social Services and other partners.
* To promote and identify areas requiring development within the Intensive Support Function team and d to take a role in the development of services in conjunction with the Clinical Leads, Team Operations Manager, Head of Clinical Services and other professionals.
* To take ownership of the management and coordinator of complex cases and supporting with the implementation of clinical pathways
