A rare and exciting opportunity has arisen for an experienced and motivated Community Mental Health Nurse (RMN) within the Younger Persons Memory Service. This post is based within our Older Adult Community Mental Health Team (CMHT) in Queen Margaret Hospital, Dunfermline. In applying for this role, you will be a mental health nurse registered with the Nursing and Midwifery Council.

The post holder will take a collaborative role in the assessment of cognitive difficulties in adults under 60, working in partnership with neurologists, psychiatrists, psychologists, and allied health professionals throughout the diagnostic process. Where a diagnosis of dementia is confirmed, the nurse provides ongoing, coordinated input and support for the individual and their family throughout the dementia journey, including end-of-life care. If a diagnosis of dementia is excluded, the nurse will ensure appropriate signposting and discharge/follow up to relevant services which may include primary and/or secondary care services and/or third sector organisations.
The successful applicant will join a small, supportive multidisciplinary team dedicated to holistic assessment, accurate diagnosis and high-quality individualised care for younger people experiencing cognitive issues. Further to a diagnosis of dementia, you will work in partnership with multidisciplinary team colleagues within the Older Adult Mental Health Team in providing post diagnostic support and ongoing high quality, person-centred care, treatment and assessment for people affected by young onset dementia.

We would particularly welcome applications from compassionate and skilled individuals with post-registration experience in community mental health, cognitive assessment and dementia care. A sound knowledge of the 8 Pillars model of community support in dementia, supporting early intervention and aiming to build resilience of young people diagnosed with dementia and their families/carers would be an asset.

We are seeking a confident individual who has the skills to support risk assessment and management in often sensitive and/or challenging situations on a lone working basis. You will work in patients homes predominantly within West Fife and within clinic settings, with a requirement to join the Younger Persons Memory Service multidisciplinary team in clinic within Victoria Hospital, Kirkcaldy on a regular basis. You will hold your own caseload and be expected to complete mental health and risk assessments, implementing and evaluating person centred care plans. You should possess excellent team working and leadership skills with the ability to motivate others and use your own initiative. You should demonstrate excellent communication skills and be adaptable to change along with evidence of your own continual personal development.

You will assist with the supervision and development of junior staff members within the Older Adult Community Mental Health Team as necessary and will be expected to provide Assessor/Supervisor role for student nurses on a regular basis. You will be required to support the Nurse Team Leader with delegated managerial tasks. There is a need to use IT systems such as Microsoft Office and Morse.

Successful applicants will receive a full induction, and opportunity to participate in clinical and nurse line management supervision.

IT IS VITAL THAT THE SUCCESSFUL CANDIDATE CAN TRAVEL ACROSS A LARGE GEOGRAPHICAL AREA IN A TIMELY MANNER.

Working pattern The service operates within office hours Monday to Friday 0900-1700.
