LCHS has an opportunity for Registered Nurses to join their South Lincs Community Nursing Team.
We have vacancies for 2 full time positions.
Role Overview
As a community staff nurse you will work autonomously and as part of a team, delivering care to patients in their own homes, care homes and clinic settings. Successful candidates will assess care needs, and plan, deliver and evaluate nursing care using innovative evidence‑based practice.
Community nursing is exceptionally rewarding. It allows therapeutic relationships to be developed with patients in their own homes and embodies the person‑centred care approach.
Skill Set
Your role will include wound management, continence care, palliative care, medication administration, long‑term condition management and close collaboration with the MDT (Social Workers, GP's and Allied Health Professionals).

Learning and Development
We offer a 12‑week induction programme, robust preceptorship and clinical skills training. We also provide an earn‑while‑you‑learn apprenticeship programme.
Working Conditions
Travel to patients' homes and clinic settings is required; a personal vehicle is essential due to limited public transport.
Hours run from 8:00 am to 6:30 pm, 7 days a week, over variable shifts: Early (8:00 am‑4:00 pm), Mid (9:00‑5:00 pm), Late (10:30 am‑6:30 pm).
The role offers flexible working – part‑time, compressed hours, job shares and hybrid models – where feasible.
