Overview
We are looking for a talented individual who is keen to learn and be a proactive member of the Urology team within Hywel Dda. You must be confident and able to demonstrate excellent verbal, written and IT communication skills, as well as good organisational and planning skills. In this rewarding role, you will work closely with our clinical nurse specialists (CNS), consultants, and wider multidisciplinary team (MDT), to support the delivery of safe, effective, and patient-centred care across a variety of outpatient and diagnostic settings. The Assistant Practitioner role is pivotal in supporting the safe and holistic care of our patients and involves providing telephone and face to face advice to patients, families and carers. Under the supervision of the Urology CNS Team, this person will also be responsible for providing follow up support for patients and families who are living after treatment or with a cancer diagnosis. This will involve signposting to other hospital and community services.
Responsibilities
* Provide clear, compassionate explanations of tests, procedures, and clinic processes.
* Offer reassurance, practical advice, and signposting to specialist services or resources.
* Support patients living with urology conditions, including those undergoing cancer and long-term condition pathways.
* Clinical Documentation & Administrative Duties: Accurately update electronic patient records, documenting assessments, observations, and outcomes. Support management of clinic flow, appointments, and coordination with multidisciplinary colleagues. Assist in monitoring stock levels, equipment maintenance, and ordering supplies.
* Patient Assessment & Clinic Support: Undertake delegated clinical assessments, observations, and holistic reviews under the supervision of a registered practitioner. Prepare patients for a range of urology outpatient procedures (e.g., flow rate tests, bladder scans, flexible cystoscopy pre-assessment, prostate assessment clinics).
* Diagnostic Testing & Clinical Procedures: Perform and record measurements such as uroflowmetry, post-void residual bladder scanning, urinalysis, and other point-of-care tests. Assist registered clinicians during investigations and minor procedures, ensuring patient comfort, dignity, and safety. Prepare clinical areas, equipment, and instruments in line with infection prevention & control standards.
