Responsibilities
* Work with adults who have complex social care needs.
* Manage a varied caseload with increasing complexity and autonomy.
* Contribute to safeguarding, assessments, care planning, and service delivery.
* Collaborate with health and care partners to deliver personalised services.
* Promote independence and enable older people to live fulfilling lives using a strengths based approach
* Work closely with service users, families, providers, and healthcare professionals to support safe, independent living in the community.
