East Boro is looking for a dedicated and experienced Care Team Leader to lead and support a team of Care and Support Workers. You will provide hands-on care, management support, and supervision, acting as a role model for person-centered care and best practice.
This role involves actively participating in the care staff rota, including sleep-in duties and participation in the out-of-hours on-call system and contributing to high-quality, safe, and compliant care for all service users.
Key Responsibilities
* Lead, support, and supervise a team of Care and Support Workers, providing coaching, appraisals, performance management, and day-to-day guidance.
* Act as a role model for the Skills for Care Code of Conduct and person-centered care principles.
* Maintain up-to-date knowledge of service users’ needs and ensure care and support plans are individualized, holistic, and regularly reviewed.
* Carry out risk assessments and enable service users to take positive risks while promoting independence.
* Plan and support mealtimes, ensuring nutritional, balanced, and personalized options.
* Administer and oversee medication safely and accurately, including storage and competency checks.
* Respond appropriately to accidents, incidents, safeguarding concerns, and complaints, escalating where necessary.
* Support recruitment, induction, supervision, and training of staff.
* Maintain electronic and paper records, participate in audits, and support continuous improvement initiatives.
* Collaborate with external healthcare professionals and involve significant others in care planning where appropriate.
* Ensure the service environment is safe, welcoming, and reflective of person-centered care principles.

About you
* Knowledge of CQC Fundamental Standards for personal care services
* Ability to deliver care to people with complex needs, promoting dignity, respect, and self-directed care
* Knowledge of the Mental Capacity Act 2005, Deprivation of Liberty Safeguards, and safeguarding procedures
* Good interpersonal, verbal, written communication skills, and IT literacy
* Ability to train, coach, and develop staff
Experience
* Providing direct personal and social care, including key working and writing support plans
* Leading or supervising a team in a care setting
* Supporting people with physical, emotional, or complex health needs
* Experience with electronic care and medication management systems is desirable
* Experience in a CQC-registered service and medication administration training is desirable
