The North Middlesex University Hospital NHS Trust are currently searching for an experienced Advanced Clinical Practitioner (ACP) to join their highly skilled team.

Hours: 37.5 hours a week.

The North Middlesex University Hospital NHS Trust is a medium-sized acute and community trust with over 525 beds and over 600 community staff, serving more than 600,000 people living across Enfield and Haringey and the surrounding areas, including Barnet and Waltham Forest.

Job Summary:

The role of the Advanced Clinical Practitioner (ACP) is relatively new. Working as part of the multidisciplinary team, they make an important contribution. The role encompasses undertaking assessment of patients attending our RDC (Rapid Diagnostic Centre) clinics using a range of different assessment and management methods, which may include physical examination, ordering and interpreting diagnostic tests, prescribing medicines (subject to holding a non-medical prescribing qualification) and advanced health needs and to provide high quality clinical assessment and intervention and treatment for a wide variety of patients presenting to NMUH.

To work autonomously at an advanced clinical level in the direct delivery of high-quality care, making critical clinical decisions based on thorough clinical assessment and interpretation of diagnostic interventions and delivery of therapeutic interventions.

The post holder will demonstrate sound clinical decision making and deliver expert care for a wide variety of patients presenting to the NMUH with undifferentiated diagnoses.

To provide clinical leadership and support within the medical, nursing and allied health teams and support the Corporate Lead ACP in developing and delivering the Trust advanced practice strategy and vision.

Duties and Responsibilities:

1. Assessment and care planning to meet health and wellbeing needs.
2. Ensure that patients receive high quality clinical care including formulating a working medical diagnosis for patients sometimes presenting with undifferentiated urgent or emergency health care problems.
3. Ordering and interpreting diagnostic tests.
4. Request initial laboratory and radiological tests as required.
5. Making referrals to other health care professionals.
6. Prescribing medicines/PGDs and treatments.
7. Ensure that effective discharge planning and health promotion is delivered.
8. Ensure accurate, contemporaneous and comprehensive documentation.
9. Communicate effectively with patients, relatives, and colleagues.
10. Participate in nursing audit, taking action to address improvements required.
11. Liaise with local community, social services, paramedics and voluntary organisations in order to develop existing services for clients, carers and health professionals.
12. Liaise with the MDT and outside agencies to ensure appropriate and timely discharge/follow-up arrangements are in place for patients.
13. Accurately identify those vulnerable adults at risk of abuse.

Managerial responsibilities:

1. Participate in the recruitment and orientation of new staff.
2. Take appropriate action in dealing with situations that may result in disciplinary action.
3. Participate in the investigation of Adverse Incidents and Complaints.
4. Ensure the formal reporting and recording of adverse incidents in line with Trust policy.
5. Utilise and maintain information systems to aid required audit and to provide regular analysis reports.
6. Maintain records and send reports (e.g. police reports) set by various bodies pertaining to the specific specialty.
7. Ensure the best use of available resources within agreed budget to provide a cost-effective service.

Important Requirements:

* NMC Registration
* At least 6 months NHS experience in the last 3 years
* Strong clinical knowledge, good initiative, and an ability to work calmly and efficiently under pressure
* Excellent communication skills, both written and verbal
* Honesty and integrity, and a caring patient-centred approach
