Young Person’s Services Manager (18–30) – Hertfordshire Area
Concept Care Solutions is a trusted, CQC-registered care provider with over 20 years of experience delivering outstanding care across children’s, adult, and young people’s services.
We’re now looking for an experienced and motivated Young Person’s Services Manager to lead and grow our 18–30 Supported Living and Community Care Service across Hertfordshire. This is an excellent opportunity for a proactive and compassionate care professional ready to take the next step in management.
The Role – What You’ll Do
As our Young Person’s Services Manager, you will:
* Manage and coordinate care for young adults aged 18–30 through local authority and private contracts.
* Develop and oversee weekly support rotas for mentors and service users.
* Ensure high standards of care planning, support plans, and risk assessments.
* Liaise effectively with Local Authorities, Social Workers, Commissioners, and Brokerage Teams to support smooth transitions from Children’s to Adult Services.
* Supervise and support mentors — managing recruitment, induction, training, and compliance.
* Conduct assessments, reviews, and spot checks to ensure the highest quality of care delivery.
* Manage mentor rotas, sickness, annual leave, and disciplinary procedures when required.
* Maintain up-to-date digital records (SharePoint/Birdie systems).
* Participate in the on-call rota and cover emergency care calls when necessary.
* Work closely with the Children’s Services Manager and Senior Leadership Team to develop, expand, and promote the department.
About You – What We’re Looking For
You’re a strong leader who brings empathy, professionalism, and structure to your work. You’re passionate about supporting young adults to achieve independence and thrive.
Essential:
* NVQ/QCF Level 3 in Health & Social Care (or working towards it)
* Proven experience in domiciliary care, supported living, or young people’s services
* Excellent leadership, organisational, and communication skills
* Strong IT skills (Word, Excel, Outlook, SharePoint; Birdie preferred)
* Full UK driving licence and access to your own vehicle
* Flexibility to work on-call (evenings and weekends
Desirable:
* Experience supporting young adults through transition from Children’s to Adult Services
* Knowledge of Hertfordshire County Council care frameworks and compliance requirements
