The Paramedic will provide high quality clinical assessment and management for patients across North Sedgemoor PCN. The role combines same day acute clinics, home visiting and support for our Enhanced Health in Care Homes service. The post holder will contribute to multi disciplinary team meetings, help coordinate safe follow up and work closely with GPs and advanced practitioners. This is a permanent network wide role across practices and care settings in North Sedgemoor.

A full UK driving licence and access to a vehicle for work are essential. We are a learning organisation and provide funded development that may include First Contact Practitioner training, Masters level modules and Independent Prescribing where appropriate. Main duties and responsibilities Deliver same day assessment and treatment clinics within scope of practice Undertake home visits for urgent assessment, planned review and follow up Support Enhanced Health in Care Homes including ward rounds, care planning, soft sign monitoring and escalation of deterioration Take part in multi disciplinary meetings. Present concise updates, agree actions and complete follow up Request and interpret appropriate investigations within competence and arrange safe next steps Provide clear safety netting, self care advice and escalation to senior clinicians when required Prescribe if qualified or work to local directions and protocols where appropriate Maintain accurate and timely records in clinical systems.

Support coding, audit and service improvement Contribute to population health and prevention work that reduces health inequalities Communicate professionally with patients, carers and partner services. Promote person centred care and shared decision making Work within local pathways, national guidance and PCN standard operating procedures Support induction and informal teaching for colleagues where appropriate Professional development and supervision Participate in a structured induction with a named supervisor Engage in regular clinical supervision, case review and peer learning Maintain up to date continuing professional development and revalidation requirements Access funded development opportunities aligned to scope of practice which may include First Contact Practitioner training, Masters level study and Independent Prescribing subject to suitability Governance and organisational responsibilities Adhere to all PCN and practice policies including confidentiality, information governance, equality and diversity, infection prevention and control, health and safety and safeguarding Contribute to significant event review, clinical audit and quality improvement Report incidents and near misses promptly and support a learning culture Work flexibly across multiple sites. Occasional evening or weekend sessions may be required to meet service need
