Paediatric Transition Diabetes Specialist Nurse (PDSN)

Harrogate Hospital has an exciting opportunity for an experienced nurse to join the Children’s Outpatient Department as Paediatric Transition Diabetes Specialist Nurse. The Paediatric Outpatient Department and Children’s Diagnostic Centre delivers high quality, safe, and effective care for children and families, offering a range of consultant, nurse and allied health professional clinics. The department assesses and treats patients with various conditions in a timely manner and serves as a source of advice and expertise for other Trust services.
Main duties of the job
Scope and design the clinical transition pathway for young people aged 16‑19 years embarking on transition to adult services in collaboration with MDT members.
Provide specialist and advanced advice and support to multi‑professional teams to develop the transition pathway and improve the service.
Support and facilitate the transition strategy for diabetes with medical, nursing and other staff groups across the Trust to ensure a positive experience for young people aged 16‑19 years.
Ensure a clearly defined transition pathway for all young people with diabetes, ensuring a supportive, smooth and seamless transition to adult services.
Demonstrate improved clinical outcomes for young people with diabetes by developing best practice to meet their specific needs, reduce variation and reduce risk for transfers between paediatric and adult services.
* In collaboration with the paediatric diabetes MDT, actively advocate for young people in their preparation for transition and during the transition period to adult services.

Person Specification
Criteria
* RSCN/Children's Nurse
* Postgraduate Paediatric Diabetes Degree
* Postgraduate non‑medical prescribing
* Evidence of ongoing professional development in the area of specialism
* Substantial experience in paediatric diabetes care as a PDSN
* Previous experience in a leadership role
* Experience working within the community
* Experience in safeguarding children
* Quality Improvement knowledge
* Regular attendance at quality‑of‑care meetings/governance meetings
* Experience of transition services (paediatric to adult care)
* Regular attendee at national level focus groups for children with diabetes

Salary
£49,387 to £56,515 a year (pro‑rata).
