Social Worker Integrated Triage & Assessment Team

up to £37.35 via Umbrella
A Social Worker is required to join Ceredigion Social Services Triage & Assessment Team for an ASAP start. This is an initial 3-6 month contract with opportunity to extend. Pay rate for this post is up to £37.35 per hour umbrella. This is a Monday-Friday 9-5 post, 37 hours per week.
Role focus
This team is a front door team focused on Adults and Carers, so experience in working with Adults is essential. The role includes undertaking assessments in both the community and hospital settings. The worker needs to be able to undertake visits in the county – both planned and at short notice. Candidates will need to have a good understanding of key legislation and regulations for both adults and children due to the county operating a through-the-ages model. As a senior practitioner you will be allocated more complex cases to undertake the client and family assessment for both adults and children, with a particular focus on prevention. A good understanding of the Mental Capacity Act (2005) is required and the ability to undertake capacity assessments and best interest meetings.
Integrated Triage & Assessment Services
The Integrated Triage and Assessment Services consists of three elements:
* Sift
* Triage
* Assessment
Sift
Acts as the gatekeeper for all enquiries including Clic and electronic referrals. Sift will review all referrals, gather additional information in order to make a decision on next steps such escalation to Porth Cynnal (Safeguarding), de-escalation to Porth Cymorth Cynnar, or assignment to Porth Gofal for proportionate integrated assessment. Sift will also provide information and advice where appropriate to support the family self-resilience where no further action is required.
Triage
The integrated Triage will undertake a proportionate assessment using the Signs of Safety model to identify the person and their family’s worries and strengths and agree a plan to resolve their worries. Further assessment may be identified following Triage enquiries.
Assessment
Workers will undertake a client and family assessment if required using a whole family approach. The focus will be one of recovery for adults whereby a recovery plan will be developed to support the family to regain their independence.
Qualifications & Experience
To apply for this role, you will need to hold -
* A minimum of 6 months experience of working in the UK as a registered social worker
* A relevant Social Work Qualification
* Registration with Social Care Wales / Social Work England
* Enhanced DBS, registered on the update service or be happy to apply for a DBS
* 3 Years of referencing
* Driving license with access to a car due to the remote geography of the county
* Welsh speaker would be very beneficial but not essential
