Overview
Job Summary: We are looking for a dynamic and proactive Clinical Pharmacist to join our expanding team. You’ll play a key role in medicines optimisation, structured clinical medication reviews, and improving prescribing safety across our network. Working closely with GPs, nurses, and fellow pharmacists, you’ll contribute to enhanced patient outcomes, reduce health inequalities, and provide specialist medicines advice within a supportive and forward-thinking environment.
Key Responsibilities
* Conduct structured clinical medication reviews (including care homes and complex polypharmacy cases)
* Provide patient-facing support via clinics and telephone consultations
* Perform medicines reconciliation following hospital discharge or care transitions
* Advise on and implement medicines safety initiatives and QI projects
* Contribute to PCN prescribing audits, MHRA alerts, formulary reviews, and national guidance implementation
* Support education and training of the primary care team
* Help improve population health and reduce inequalities through medicines optimisation strategies

Job Description / Responsibilities
The main duties and responsibilities to be undertaken by the post holder may include any or the entire following list. Duties may be varied from time to time under the direction of the partners and senior management team and will be dependent on current and evolving practice workload and staffing levels.
* See patients with single or multiple medical problems where medicine optimisation is required (e.g. COPD, asthma). Review the ongoing need for each medicine, monitoring needs and support patients with their medicines ensuring best use of medicines (medicines optimisation). Make appropriate recommendations to Senior Pharmacists or GPs for medicines optimisation or actions as appropriate
* Undertake structured clinical medication reviews with patients, including Care homes and make appropriate recommendations to Senior Pharmacists or GPs for medicines optimisation or actions as appropriate; work with care home staff to improve safety of medicines ordering and administration
* Provide patient-facing clinics for questions, queries and concerns about medicines
* Provide a telephone helpline for patients with questions about their medicines
* Answer medicine-related enquiries from GPs, practice staff, other healthcare teams and patients; suggest solutions and provide follow up
* Review medicines related to unplanned hospital admissions and readmissions; implement changes to reduce prescribing to high risk groups
* Reconcile medicines after transfer of care and ensure continuity of supply for high risk groups
* Ensure appropriate referrals to healthcare professionals and timely follow up
* Identify cohorts at high risk of harm from medicines and use data to support decision making
* Contribute to development and implementation of new services with medicinal components
* Analyse and present medicines data to highlight issues and risks at PCN/GP Practice population level
* Undertake clinical audits of prescribing and implement changes; respond to MHRA alerts and national guidance
* Provide education and training on therapeutics and medicines optimisation
* Support PCN/CQC standards compliance where medicines are involved
* Support public health campaigns and population health improvements to reduce health inequalities
Qualifications & Experience
Essential
* Undergraduate degree in pharmacy
* Experience working as a clinical pharmacist
* Hold or be working towards an independent prescribing qualification
* Professional registration with the General Pharmaceutical Council
Experience
* Experience working as a clinical pharmacist
Desirable
* Experience of working in a primary care environment
* Experience or a special interest in diabetes management
