Job summary

The successful individual will need a Foundation Degree in Health and Social Care and Literacy and Numeracy Level 2 or equivalent to be recruited into the position on Band 4.

An exciting position has become available for a Band 4 Associate Practitioner to join Central Durham Adult Community Mental Health Team based at Kirkstone Villa, Lanchester Road Hospital site.

You must have the Right to Work in the UK in order to be successfully appointed to this role. Please note, this role does not meet the required eligibility criteria for sponsorship under the Skilled Worker visa and therefore the Trust is unable to offer sponsorship for this particular role.

Main duties of the job

This role is to support a busy adult mental health community team. The successful candidate will work as a key worker for adults requiring treatment and intervention for an identified mental health concern.

We will provide you with clinical caseload and managerial supervision, a thorough induction and an extensive range of training opportunities relevant to the role.

Job description

Job responsibilities

To improve the lives of people with mental ill health by minimising the impact of their condition through the delivery of excellent services to promote recovery and well being.

To be compassionate in meeting the needs of patients and their carers.

To work as part of a community team which may be integrated, workingcollaboratively and in partnership with local authority services.

To deliver patient care, as directed by registered practitioners and the team manager, undertaking a range of clinical/therapeutic interventions with limited or indirect supervision in patients homes and other community settings.

To promote social inclusion, community access and participation through the provision of patient care.

To show commitment to and participate in quality improvement activities.

To promote at all times a positive image of people with mental ill health.

Qualifications

Essential

4. Foundation Degree in Health and Social Care
5. Key skills in literacy, numeracy and ITQ level 2 (or equivalent)

Experience

Essential

6. Previous experience of working with people with mental/social health needs or learning disabilities
7. Previous experience as a Band 2/3 HCA or equivalent in a care environment.

Knowledge

Essential

8. Person centred care
9. Evidence based knowledge of current practices in the field.
10. Awareness of legal frameworks including Mental Health legislation

Skills

Essential

11. Effective verbal and written communication skills
12. Time management and ability to prioritise effectively

Desirable

13. Competency in venepuncture
