* Provide personalised care through continuity and follow‑up where appropriate.
* Undertake a weekly care‑home session and liaise with care‑home staff to ensure coordinated care.
* Participate in the Thursday and Friday afternoon on‑call rota, managing urgent queries, same‑day appointments and clinical decision‑making.
* Prescribe medications safely in accordance with local and national guidance.
* Make appropriate referrals to secondary care and community services.

Administrative Responsibilities

* Complete clinical administration including reviewing test results, handling Docman correspondence and updating records.

* Support effective communication between the practice, community teams and external providers.

Training & Development

* Engage in ongoing CPD, appraisal and revalidation.
* Contribute to teaching and supporting medical students, GP trainees or other learners where appropriate.
* Participate in mandatory training and practice training sessions.

Professional Standards

* Work in line with GMC guidance, NICE standards and practice/PCN protocols.
* Maintain patient confidentiality and data security under GDPR and Caldicott principles.
* Operate within health and safety, safeguarding and infection control frameworks.

Working Pattern

* 5.5 sessions per week: Wednesday, Thursday, Friday.
* Includes clinical work, care‑home session, admin session and two half‑day on‑call sessions.
