Join to apply for the Mental Health Practitioner role at Oxleas NHS Foundation Trust
As part of our mental health team providing care in a prison setting, you will deliver specialist mental healthcare to offenders and work within the Mental Health In-Reach Team and wider mental health services. You will be required to work in a psychologically minded way to help offenders achieve their agreed goals and quality health outcomes. You will manage a mixed and challenging caseload, including offenders with learning disabilities and mental health conditions, and contribute to alternatives to inpatient admission and early discharge through high-intensity interventions and complex case management.
Our Mental Health Team delivers specialist interventions at primary and secondary care levels, including referral management, screening assessments, triage, evidence-based interventions, care planning and risk assessment, plus one-to-one and group-work facilitation. You will work to ensure continuity of care, sharing information with community mental health teams as appropriate and supporting the Care Programme Approach for all offenders as necessary. Our aim is to reduce length of stay and ensure effective continuity of care between prison services and external NHS/independent in-patient services.
Oxleas offers NHS employment with benefits and opportunities for career and personal development in a forward-thinking service that values clinical and interpersonal growth.
