We are excited to offer a brand-new opportunity for three Social Care Coordinators to join our Adult Social Care team as part of an innovative Primary Care Network (PCN) pilot. These roles will help shape a new way of working that strengthens collaboration between health and social care across the East Riding.
This is an internal-only opportunity for colleagues who are motivated by change, partnership working, and improving outcomes for adults in our communities.
About the Role
As a Social Care Coordinator, you will provide accessible, place-based adult social care within selected East Riding PCN practices. Working across both GP practices and ERYC offices, you will:
1. Offer immediate advice, guidance and signposting on local services, support options, financial guidance and community resources.
2. Complete Care Act assessments and develop personalised care plans using a strengths-based approach.
3. Make timely referrals to other support services to help residents maintain independence and meet the prevent–reduce–delay agenda.
4. Work proactively with adults—sometimes in crisis—to help stabilise situations and reduce risks.
5. Build strong working relationships with PCN colleagues, adult social care teams and wider partners.
6. Maintain accurate, timely case records on the adult information database, contributing to essential performance and outcomes data.
This is a developing role, and you will be encouraged—and supported—to influence how the pilot evolves. We are looking for someone who will take ownership, bring new ideas forward and help shape our approach to integrated partnership working.
About You
We are seeking a suitably qualified, enthusiastic and resilient individual who can:

10. Demonstrate an understanding of key legislation including the Care Act 2014, Mental Capacity Act 2005, Human Rights Act, and safeguarding procedures.

12. Build positive relationships that strengthen partnership working and improve outcomes for adults.
Access to transport and a full, clean driving licence is essential due to travel between locations.
