Job Overview

This is a full‑time (10 PA) locum post. It will be based at across sites with clinics at both Stoke Mandeville and Wycombe Hospitals. Job share would be considered, 5 PAs per candidate.

The Paediatric department has been successful in getting approval to expand their consultant presence by new consultants to meet the future workload to deliver safe high quality care to all children in Buckinghamshire with a particular focus on our trust care values.

We are seeking a keen, dynamic general paediatrician with a special interest that will compliment the current team.


Core Responsibilities

* Carry out both general paediatric and neonatal duties and participate in the overnight on‑call consultant rota.
* Handle weekend evening ‘twilight’ shifts in the Paediatric ED to support the middle grades and help protect capacity.
* Join a team of consultants and undertake inpatient and outpatient work in neonates and general paediatrics including inpatients and children admitted to the observation ward.
* Attend weeks and on‑call activities on a pro‑rata basis.
* Run specialist clinics in your subspeciality; 3 clinics every week across both hospitals to ensure local access for patients.
* Organise specialist clinics and develop a service that benefits the children of Buckinghamshire; time for service development will be given in the job plan.
* Act as Paediatrician of the Week (POW) and Neonatologist of the Week (NOW) totalling 7 a year. Attend weeks for the children’s observation unit (1.5–3.5 weeks depending on rota restructure). During these days, no other clinical commitments.
* Carry out “twilight” shifts from 16:00‑22:00 within the Children’s Emergency Department on weekends – 4 weekends a year to meet college standards.
* Commit to the education of junior staff and medical students; encouraged to consider being educational supervisors for the deanery.
* Participate in clinical audit across the department in line with Trust policy and guidelines.
* Implement audit and research findings across the department and comply with the Trust clinical governance processes in a timely manner.
* Engage in supporting professional activities and continuing professional development as required of consultants.


Clinical and Rota Details

The post will be 10 PAs under the “new” consultant contract. An indicative job plan and timetable can be found in the full documentation. The final job plan will be agreed with the SDU lead and will be subject to renegotiation at least annually.

The existing general paediatric consultant rota is designed around a 45‑week rolling cycle. A planned change to a 60‑week rolling rota is considered for all posts, should they be successful. POW and NOW weeks are shared equally between 15 consultants. One-on‑call nights (non‑resident) are shared equally. A second on‑call (non‑resident) consultant covers the Children’s Day unit at Wycombe Hospital and is available at night as back‑up for the first‑night consultant in case of simultaneous neonatal and paediatric emergencies.

On a new Children’s ED department opening in April 2023, on‑call weeks Monday‑Friday between the team have been introduced. Each consultant has 10.5 on‑call weeks per year, with the NOW and POW weeks running from Friday‑Thursday. Three‑half times a year the consultant will also cover the new Children’s observation unit (COU), Monday‑to‑Friday, which is co‑located with the new Children’s ED department (this number may change to 1.5 with rota redesign).

We are currently looking at redesigning the rota with 60‑week rolling and potentially stopping second‑call overnight duties to decrease the supplement to 3 % from 5 %. Daytime responsibilities for second‑call at Wycombe Hospital will continue. Eight consultants share weekday evening twilight shifts, with fixed days covered across the year by paired consultants doing 26 shifts per year. Each of the eight consultants also cover 2.3 twilight weekends. This post is within the latter group of seven consultants. The combined on‑call shifts mean the rota is 1 in 7.5, attracting a Category A – Medium Frequency (5 %) on‑call supplement, which may change to 1 in 15 category A‑low frequency (3 %) with the above changes.

Leave Arrangements

Leave should be arranged by mutual agreement of consultant colleagues and approval of the clinical director and/or general manager, in accordance with standard Trust and NHS regulations. It is essential that six weeks’ notice is given to allow for proper planning and prevent cancellations of patients’ appointments/surgery. The post holder has a responsibility to ensure that appropriate arrangements are in place to cover his/her clinical duties during any absence and that all leave is coordinated with colleagues to ensure adequate cover arrangements, including all forms of leave.
