Job Overview
This is a very exciting opportunity for an RMN to join our team and enable our mental health function to expand. The new post holder would be supported by our existing Mental Health Practitioner in delivering assessment and care planning for our patient population who present with mild to moderate mental health issues. They would be working closely with our GPs/ ANPs, the wider primary care / community teams as well as with other mental health services including the PCPT and the CMHT as required.
We would welcome applicants with a broad range of post qualification experience within mental health services. Primary care and/or community experience would be particularly advantageous.
Post Overview
1. The post holder will assess patients who have been triaged by the Mental Health Practitioner, but may also see patients referred directly by GPs/ ANPs. It would be expected that the post holder would be competent in risk assessment.
2. The post holder will provide mental health assessment and implement/ evaluate an appropriate treatment plan/ intervention/ recommend onward referral, seeking support/ guidance as required.
3. The post holder is responsible for their own time management on a day to day basis
4. The post holder will be expected to be able to deliver a range of interventions including brief mental health / interventions, health promotion and psychoeducation
5. The post holder must be familiar with and compliant with all mental health, community care and child health legislation or professional guidance pertinent to care delivery.
6. The post holder must maintain written and electronic patient records, working within the parameters of local policies, professional guidelines and national data protection legislation.
7. The post holder will be expected to contribute to evaluation, and any associated quality improvement, audit and research activities undertaken by the practice.
8. The post holder will have excellent communication, IT and organisational skills.
Hours
Hours are negotiable up to fulltime
Salary
Salary is dependent on experience and we expect this to be guided by an AfC Band 5 salary equivalent.

Our current clinical team consists of 4 GP partners, 2 ANP partners, one an experienced Elderly Care Advanced Nurse Practitioner and one an experienced Acute Advanced Nurse Practitioner who also does Duty doctor sessions as well as 12 salaried GPs. We also have a fantastic Management Team, team of Pharmacy Technicians and Primary Care Pharmacists.
