Randstad is currently recruiting for a dedicated Social Worker to join the Hospital Discharge Team at Manchester City Council. About the Role
As a Social Worker in the Hospital Discharge Team, you will work closely with health professionals, families, and community services to coordinate discharge planning. Your role is to ensure that individuals leaving hospital receive the right care and support to recover safely, reduce hospital readmissions, and promote long-term wellbeing.
Key Responsibilities
Undertake Care Act assessments in hospital settings or virtually.
Collaborate with NHS colleagues to support safe and timely discharges.
Develop person-centred care and support plans, involving service users and their families in decision-making.
Arrange appropriate packages of care, residential placements, or community-based support where required.
Assess mental capacity where relevant and complete best interest decisions.
