What you will do:
* Conduct comprehensive mental health assessments for older adults within the community
* Develop and implement personalised care plans in collaboration with multidisciplinary teams
* Provide advice and support to clients and their families, ensuring their needs are addressed
* Monitor and review ongoing progress, adjusting interventions as required
* Maintain accurate records and documentation in line with organisational and legal requirements
* Work collaboratively with colleagues to deliver effective mental health services
* Advocate for the mental health needs of older adults within the community
