Qualified Social Worker - Hospital Discharge

About the role: We are looking for a dedicated and passionate Qualified Social Worker to assist with Hospital Discharges in a local authority in South Wales, on a initial 3 month contract.
Responsibilities
* Conduct thorough assessments to identify patient and family needs
* Develop personalised discharge plans that address patients’ unique circumstances
* Collaborate with medical professionals, nursing staff, and community resources to coordinate care
* Provide emotional support and counseling to patients and their families
* Assist in the arrangement of post-discharge services and follow-up care
* Advocate for patients’ rights and access to appropriate services
What you need
* A degree in Social Work or equivalent
* Registered with Social Care Wales
* Completed relevant post-qualifying training
* Enhanced DBS check
* Excellent communication and written skills
