We are recruiting for a Health and Wellbeing Practitioner at HMP Send.
Here at Forward Trust, we deliver a complex range of drug and alcohol services in the unique prison environment. Our support includes providing advice, health and wellbeing, motivational work, clinical services, and a wider range of group work and treatment programmes.
The Forward Trust services which are delivered within prison settings are commissioned by NHS England and are delivered in partnership with primary healthcare providers and HMPPS. Integration and partnership are integral to the work we do.

Role Responsibilities
The Forward Trust provides both clinical and psychosocial Substance Misuse Services across Surrey Prisons in partnership with NHS Trust healthcare providers. Each service operates a Health and Wellbeing framework taking a trauma-informed and gender-responsive approach to delivery of a wide variety of interventions and integrated pathways tailored to meet specific needs of each prison and individual service users. With proactive partnership working being a key aspect, delivering these services involves multidisciplinary collaboration across mental health, healthcare services, and the prison environment.
You will be based on-site, working within a team that has a broad skills mix to deliver combined psychosocial and clinical interventions, providing recovery-oriented Health and Wellbeing Services to service users aged 18 and over at HMP Send.
This involves working directly with detained individuals who may present with low-level mental health needs and/or substance misuse issues. The service operates 7 days a week, so weekend working will be required on a rota basis.
Responsibilities include:
* Managing a caseload, providing harm reduction advice and guidance
* Conducting assessments and formulating care plans in co-production with service users
* Delivering structured 1:1 sessions and facilitating group work
* Performing second signatory duties
* Planning for release and ensuring transitional support for individuals leaving the prison and returning to the community
* Accurate and timely recording of data to measure effectiveness
The Ideal Candidate
Essential skills and experience include:
* Experience working within mental health or substance misuse services
* Strong IT skills
Desirable skills include:
* Experience in comprehensive assessments and SMART care planning
* Delivery of structured interventions and facilitation of group work
* Use of motivational interviewing, CBT, and motivational counselling skills
* Knowledge of issues faced by those with substance dependency or low-level mental health issues
* Understanding of the Recovery Agenda and national drug strategies
* Knowledge of the 12-step treatment approach and experience working in such programmes
* Experience within a Health and Wellbeing framework
* Personal experience of addiction or imprisonment (minimum 3 years in recovery for addicts, or released at least 5 years ago for prisoners)
