Are you driven to make a real difference in people’s lives from patients’ own homes? We’re looking for a compassionate and proactive Band 6 Deputy Sister/Charge Nurse to join our well-established York Community Response Team.
Responsibilities
* Deliver high-quality holistic care to prevent unnecessary hospital admissions, alongside specialist nursing interventions
* Support timely discharges and promote recovery at home
* Collaborate with a multidisciplinary team including Physiotherapists, OTs, Nurses, and Support Workers
* Engage patients and families using health coaching techniques to set realistic, empowering goals
* Coordinate activities in the CRT team and ensure effective communication; provide support to registered and unregistered staff within the ward/department
* Assess patient care needs and contribute to the development, implementation, and evaluation of care programmes
* Promote quality evidence-based practice, support the nursing team, and manage the ward/department in the Team Leader’s absence
* Work in partnership with the Team Leader and team to maintain high standards of nursing care
* Provide day-to-day support to non-registered clinical support staff within the York Community Response Team
