Job Title: Mental Health Link Worker
We are seeking an enthusiastic and motivated Mental Health Link Worker to join the Primary Care Network (PCN) at a GP practice in the Hindley area. The PCN serves as the first contact point for individuals seeking mental health support, especially those who may not meet the criteria for traditional mental health services.
The team includes a Mental Health Practitioner, an Assistant Psychologist, and works closely with GPs. Your role will involve supporting individuals in community settings to improve their health, wellbeing, and quality of life by connecting them with mental health services, voluntary sector organizations, and community activities.
Key Responsibilities:
* Support individuals to access appropriate mental health and community services.
* Work within primary and secondary care settings to facilitate early support.
* Provide knowledge and guidance about mental health services to colleagues.
* Assist individuals in navigating mental health and community resources based on their needs.
* Deliver education and training to staff, volunteers, and peer support workers.
* Participate in multidisciplinary team meetings to discuss patient care.
* Collaboratively set goals with clients and plan evidence-based interventions to promote recovery and social inclusion.
* Use coaching and motivational interviewing techniques to support clients.
* Manage a caseload of clients, aiding them in connecting with community support.
* Collaborate with volunteer community connectors and peer support programs.
* Maintain accurate electronic records in line with data protection standards.
* Contribute to clinical discussions and provide reports on client progress.
* Participate in team activities, including assessments and follow-up clinics.
