Integrated Primary Care Nurse – HMP Isis
Oxleas Forensic & Prisons Directorate seeks a registered nurse to deliver high-quality healthcare to adult and young offenders at HMP Isis, as part of our Integrated Primary Care Service to Greenwich prisons.
Responsibilities
* Support senior staff to develop a clinically effective, patient‑centred primary healthcare service, including acute & crisis care, primary care interventions, substance misuse support, screening programmes, long‑term condition management and health promotion activities in line with national guidance and best practice.
* Demonstrate robust clinical leadership to junior staff, ensuring evidence‑based nursing practice and a proactive approach to quality and outcome‑driven care.
* Work within a challenging, multi‑agency environment as part of a multidisciplinary team that includes nursing, medical, discipline staff and allied health professionals such as dentistry, optometry, physiotherapy and podiatry.
* Play a lead role in the ongoing development and delivery of an effective integrated healthcare service model.
* Manage a clinical caseload, ensuring all clinics are delivered in a clinically effective, efficient and timely manner.
* Implement and support strategies to promote and improve health, prevent disease, and monitor and audit the quality and effectiveness of service delivery.
* Maintain effective case management to ensure continuity of care throughout an offender’s treatment pathway while detained in prison.
* Liaise with other professionals, agencies and organisations to share expertise, promote ideas and collaborate as required.
Qualifications & Competencies
* Licensed registered nurse with the appropriate qualifications.
* Preferably, prior experience working in prisons or secure settings (not essential).
* Demonstrated ability to exercise professional autonomy, make complex, critical judgments and effective decision‑making skills.
* Strong communication, organisational and multi‑agency working skills.
