Be at the heart of community care - using your clinical expertise to ensure people receive the right support, at the right time, every time. We are seeking an experienced clinician to join our Community Health Single Point of Access (SPA) for Adult Services. This role is central to ensuring safe, timely and effective coordination of care across our community health pathways. As an autonomous practitioner within a multidisciplinary team, you will provide expert telephone‑based clinical triage, using your clinical knowledge and understanding of urgent and community services to assess, prioritise and direct referrals appropriately. Your contribution will help ensure individuals receive the right care and support at the right time. Base Location and Working Hours The role will be based at The Orbital, Swindon, SN25 4AN. We are happy to consider applications for part‑time or full‑time hours, ranging from 22.5 to 37.5 hours per week. Why Apply? Key Responsibilities Deliver high‑quality telephone clinical triage and assessment within the SPA Use sound clinical judgement and diagnostic reasoning to provide treatment advice, care guidance or onward referral in line with agreed pathways Support admission avoidance and facilitate safe and timely discharge through coordinated referrals into community services Help patients navigate the wider health and social care system, providing effective signposting to VCSE organisations and community resources Promote independence and wellbeing by supporting people to remain safely at home wherever possible Work collaboratively across health and social care partners, contributing to a fully integrated approach to care Participate in a variety of shifts across extended hours, ensuring the SPA remains responsive to professionals across the system Please see job description attached for full details of the role.
